As a volunteer of Cheng Hong Welfare Service Society (“CHWSS”), I agree to treat all personal information of beneficiaries and/or donors obtained during my service as confidential (including pictures and videos of beneficiaries and/or donors taken during events, etc.) and will not collect, use or disclose such information without the permission of CHWSS.

I consent to CHWSS collecting or using the personal data I have provided in the Volunteer Application Form, and throughout my service, for the purposes of
a) adding my particulars into CHWSS’ volunteer database and using them to send me information about CHWSS’ upcoming events and volunteering opportunities;
b) to contact me for any matters relating to my service as CHWSS’ volunteer;
c) internal and/or external publicity for CHWSS’ events and activities, using photographs and/or videos taken of me; d) disclosing to the authorities or any government agencies, upon request, or as may be required by applicable law or regulation.
